World Athletics, the global governing body for track and field sports, has announced its intention to update the regulations concerning transgender athletes and athletes with differences in sexual development (DSD). This move comes amidst ongoing debates and protests regarding the inclusion of transgender women in female sports categories.
Among the proposed changes is the reintroduction of sex-verification testing, which will be conducted using a cheek swab test. This test aims to identify the presence of the SRY gene, which is typically found on the Y chromosome and is associated with male sex development. The implementation of this test marks the first time such procedures have been mandated since the cessation of similar tests by the International Olympic Committee in 1999.
According to World Athletics, the SRY gene is used as a "highly accurate proxy for biological sex". However, the organization also states that there will be provisions for additional diagnostic processes at the discretion of the athlete. This mandatory testing will apply to all athletes wishing to compete in women's international competitions.
This policy shift occurs almost two years after World Athletics implemented a ban on transgender women competing in female categories, while also imposing a testosterone limit of 2.5 nm/L for cisgender women with defined DSD. These regulations were partly influenced by similar measures adopted by other sports bodies, such as World Aquatics in 2022.
Furthermore, these decisions have been shaped by longstanding legal disputes, notably involving Caster Semenya, a two-time Olympic champion from South Africa. Semenya's legal battles with World Athletics have significantly influenced the regulatory landscape since 2012.
The latest regulatory revisions are based on the findings of a working group dedicated to examining these complex issues. This group has been active throughout 2024, making recommendations to World Athletics that include integrating DSD and transgender regulations. The proposals also acknowledge the presence of "gender diverse athletes".
A consultation period has been opened to allow athletes to express their views on the proposed changes, with the deadline set for March 5. World Athletics President Sebastian Coe has emphasized the importance of these updates, citing recent scientific advancements as a basis for ensuring fairness in female sports categories.

Despite the emphasis on competitive integrity, the proposed changes have drawn criticism from various quarters. Dr. Blair Hamilton, a trans woman and former league-level soccer player in the UK, has publicly opposed the testing regime via social media, describing it as "unjustified, unethical, and impractical".
Hamilton, along with other researchers, has co-authored a white paper challenging the necessity of mandatory genetic testing. The paper references the abandonment of such tests in 1999 due to concerns about their validity, cost, and potential to cause trauma and stigmatization among athletes.
As the debate continues, World Athletics faces the challenge of balancing scientific insight, ethical considerations, and the voices of the athletes themselves. The outcome of the ongoing consultation period and the organization's final decision will likely have far-reaching implications for the future of athletics and the inclusion of diverse athletes.
