Transgender individuals face unique challenges when it comes to family building, often rooted in transphobia and a lack of comprehensive information about trans healthcare—particularly trans fertility. Every person deserves the right to make informed choices about their reproductive options, yet for trans individuals, this journey can be fraught with obstacles.
As a queer, nonbinary midwife and author of "Baby Making for Everybody—Fertility and Family Building for LGBTQ+ and Solo Parents," I have dedicated my career to supporting trans parents and parents-to-be. Through my work and the PregnantTogether community, I have connected with hundreds of trans individuals at various stages of their family-building journeys.
The desire to become a parent is not limited by gender. Many trans individuals wish to pursue biological parenting, though they may face significant hurdles. Building a family biologically requires access to eggs, sperm, and a uterus. A trans person’s options are influenced by factors such as their medical transition status, whether they have preserved gametes, and their access to biological resources within their relationships.
Fertility preservation discussions should be a standard practice in healthcare for trans individuals considering hormonal or medical transitions. While some US insurance companies offer fertility coverage for trans individuals, many do not, making it financially difficult for many to preserve their fertility.
Pausing GAHT to pursue fertility can be a significant emotional and psychological decision. It is advisable for individuals considering this to consult with their healthcare providers and seek adequate community support. Virtual communities, such as PregnantTogether, provide crucial support networks for trans parents.
Research indicates that transmasculine individuals can have successful fertility and pregnancy outcomes. However, societal perceptions of pregnancy often impose additional social challenges, potentially increasing the risk of postpartum depression. Support and understanding from communities and healthcare providers are essential.
For transfeminine individuals, GAHT can impact semen quality, but improvements can occur if GAHT is paused. Inducing lactation is another way transfeminine individuals can participate in nurturing their children, though it requires dedication and commitment to hormonal and lifestyle changes.
The world still lacks adequate support for parents, and trans parents face additional challenges. Building a robust support system is vital for trans individuals considering parenthood. Surrounding oneself with an understanding and supportive community can make a substantial difference.
Finding affirming healthcare providers who understand and support trans family-building aspirations is crucial. It’s important to choose providers who have experience in assisting trans individuals on this journey.
Trans individuals should feel empowered to share their fertility journey on their own terms and not feel pressured to disclose personal information.
Professional mental health support is highly beneficial for those navigating both gender identity and family-building. Investing in therapy or counseling can provide invaluable support and guidance throughout this process.
