In a recent report examining the systemic issues surrounding the transition of the UK's sole youth gender clinic, concerns have been raised over the lack of a "safe handover" to newly proposed services.
The clinic at the Tavistock and Portman Centre, North London, was closed early last year. This closure was part of a strategic move to establish regional hubs across England and Wales, as recommended by the Cass Report. However, the transition has been marred by controversy and concern, with some clinicians criticizing the process as "shoddy, disorganized, and messy."
According to a report by the activist group What The Trans!? released on Monday, February 17, two former clinicians from the Tavistock clinic disclosed that there was "no safe plan" for the restructuring of services. The clinicians, speaking anonymously, criticized the handling of the clinic’s closure and the transition to new services.
The Tavistock and Portman NHS Trust, which managed the clinic, reportedly informed staff of the closure on the same day the media was notified. The trust's reasoning for not notifying staff earlier was to prevent leaks to the public. One clinician noted that the trust was aware of the closure plans "18 months before the closure date," yet staff only received a two-month notice.
Initially, the closure was delayed due to the "complexity" of the transition process, ultimately taking more than two years to complete. During this period, staff continuously raised concerns about the planned closure. They were allegedly instructed to "keep business as usual" and discouraged from asking questions.
At a subsequent all-hands meeting, staff reportedly faced threats of "disciplinary actions" if they opposed the closure. Although staff were assured they would be offered new positions at the regional hubs, this promise was not fulfilled. Several former staff members were laid off and not included in the new service structure. Clinicians expressed frustration, noting that many of those currently serving at the new hubs lack clinical experience.
The activist group What The Trans!? also raised concerns about the training materials provided to the new regional hubs. They referenced a report from last November by the Trans Safety Network, which highlighted warnings to clinicians regarding "coercion from parents and carers." An expert cited by the Trans Safety Network criticized the training materials, stating that they have "nothing to offer" to trans young people who are happy and well-adjusted, while also downplaying the "harms of transphobia."
