Growing worries about GPs denying hormone therapy

There's a rising concern among advocates about a noticeable surge in general practitioners (GPs) who are refusing hormone replacement therapy (HRT) prescriptions specifically aimed at transgender individuals. This alarming trend, often referred as a "staggering increase," has sparked a lot concern within both transgender communities and their allies.

Why hormone replacement therapy matters

Hormone replacement therapy, or HRT, consists in various hormone treatments like estrogen, progesterone, and testosterone. For many transgender and non-binary people, these treatments are vital in their journey towards gender-affirming healthcare. Feminizing HRT can lead a person experiencing greater development in breast tissue and a drop in testosterone levels. On other hand, masculinizing HRT can pause menstruation, deepen a person voice, and increase body hair. Cisgender individuals also use HRT, frequently as a treatment managing menopausal symptoms.

The unequal path in accessing HRT

Even though both groups utilize HRT, cisgender people often face less challenges in accessing these treatments than transgender individuals. Over recent year, there's been a troubling trend with GPs turning away transgender individuals from receiving HRT, even those who have been on treatments long-term. This situation appears tied recent reports, like Cass Report and UK Supreme Court's ruling on "sex" definition.

Reports highlighting hurdles faced by transgender patients

In October, TransActual released findings showing that 215 trans individuals were denied hormone prescriptions. Shared alongside QueerAF and What The Trans, over a quarter had recommendations from NHS gender clinics and 45% had been on HRT over five years. The research indicated GPs refused prescriptions due "lack training, perceived lack competence, absence policy, or personal beliefs."

The overall struggle in transgender healthcare

A survey from Healthwatch in July shed even more light on struggles transgender individuals face within UK's healthcare system. Surveying nearly 1,400 transgender and non-binary adults, many reported issues like being misgendered, interruptions prescriptions, and loss NHS records after changing gender markers. Only 32% rated their GP's gender-affirming care positively and barely 30% said their HRT prescriptions went without disruption.

Case example: GP practice stops HRT prescriptions

A troubling incident occurred at Jubilee Park Medical Partnership in East Midlands, where HRT prescriptions were stopped entirely transgender patients. This action drew sharp criticism and protests from Nottingham Against Transphobia. A 29-year-old transgender woman voiced her concern saying, "I've been dealing with severe anxiety at simply thought losing my HRT."

Ongoing push and advocacy efforts

Chay Brown, TransActual's director, brought attention ongoing issue GPs denying HRT prescriptions. Brown noted majority refusals stem reasons not connected medical necessity. He stated, "Trans people aren't getting same medication cisgender individuals receive without question."

An NHS spokesperson mentioned ongoing efforts helping GPs prescribe patients with gender dysphoria through specialized gender services primary care. This aims address problems some face obtaining timely prescriptions. Since July 2020, funding gender dysphoria services increased significantly, leading expansion adult clinics England from seven 12, along with launching five new pilot clinics.
